Who We Are

Nurse Nexus is an integrated comprehensive program involving physicians (surgeons, specialists, and primary care), patients, family members, hospital discharge teams, ancillary services (home health, DME, etc.) and pharmacists to coordinate transition of care from the hospital to the home. It is a high-touch patient-centric in-home program that will deliver high quality care with measurable outcomes.
